Please call to verify our participation with your insurance plan.

Routine eye examinations/eyeglasses/contact lenses may be

covered by your VISION plan.

Infections/"pink eye"/foreign matter in the eye and other medical

care, are not covered under VISION plans, but may be covered by

your MEDICAL plan.

Out-of-Network Benefits:

For patients who have insurance coverage in plans in which we

are not a part of, we will be glad to furnish you with a complete

receipt of services and materials should the patient choose to

pursue out-of-network insurance reimbursement.
